PROTECT YOUR LOVED ONES Patient Rights, Surrogate Decision-Making, Consent, Communication, and Documentation I am submitting a complaint regarding care, communication, and decision-making processes involving Michael at Oak River Rehab in Anderson, California. Our concern is not primarily the medication itself, but whether Michael’s wishes were respected, whether proper process was followed when he was considered unable to make decisions, and how another family member became involved in decision-making. Michael had recently been hospitalized. While there, he expressed that he wanted his son, T, involved in making medical decisions. A POLST was completed in the hospital with family present. We understood that Michael wanted to make his own decisions while able to do so, and that T was the person he wanted involved if he could no longer make decisions himself. C was present when this was discussed. Michael was then transferred to Oak River Rehab. Shortly before his death, Michael told T that staff were trying to increase his morphine and that he did not want more morphine at that time. T attempted to speak with a nurse that day but could not locate one. He also attempted to arrange communication with the physician and left his telephone number requesting an appointment or follow-up. No one contacted him. T had been visiting Michael regularly and speaking with him. The day before Michael’s condition deteriorated, T spoke with him and Michael said he was feeling better. We therefore had no reason to understand that Michael had changed his wishes or believed he was actively choosing to end treatment. We subsequently learned that C had been involved in authorizing a medication increase. We were shown documentation where C’s name was written, and she signed medication-related paperwork in our presence. Staff told us C was not “in charge,” yet simultaneously told us to contact her if we disagreed with what was happening. This was never adequately explained. We also observed that C’s name had been written on the facility face sheet/emergency contact information and on a purple POLST-type form at the top of Michael’s chart, though there was no signature on the purple form. We do not know who entered her name, when it was entered, or what authority was relied upon. The nursing supervisor told us she could not discuss Michael’s care with us because we were not listed on the emergency contact information. However, C was listed there. We do not know who placed C on the list or whether Michael provided that information, as he did not appear to complete all admission paperwork himself. We are concerned staff accepted C’s account that T and family were not involved without independently verifying it. C did not contact T regarding Michael’s deterioration, despite T being his son who regularly visited and communicated with him. There were significant difficulties reaching staff outside normal business hours and on weekends. When T attempted to address Michael’s expressed wishes, he could not locate a nurse or physician. A feeding tube was also discussed despite Michael’s POLST indicating he did not want artificial nutrition. Although not placed, this raised concerns about whether his documented wishes were being reviewed and followed. A staff member told us that morphine would “shut his system down.” We understand morphine may be used for pain or respiratory distress; our concern is whether Michael’s own wishes and proper decision-making processes were respected. We request investigation of: How and when Michael’s capacity was assessed. What procedure was followed to identify a surrogate and whether Michael’s stated preference for T was considered. Who entered C’s name on the face sheet and POLST-type form, why she signed medication paperwork, and whether she had legal authority. What efforts were made to contact T after he left his phone number, and whether appropriate after-hours/weekend communication procedures were available. Whether Michael’s POLST and other documented wishes were reviewed and followed.

I want to thank Melinda for helping me find a place she’s such a wonderful lady and without her I wouldn’t have a place to live she’s helped me with everything when my medical and everything else I need to find more than five stars 100% she’s great and all the other employees here that have helped me they’ve been great when I call 20 times a day they still come and they’re nice all the time they’re never rude and they’re always in the nurses always bring the medicine on time I love everyone here this is the best place they should get a big reward for for being the best thank you Barbara Ortega

My mom Mary was treated terrible there denied care and pushed to hard and the staff verbally abused me and my son Tyler. I have a solid legal case as everything was video recorded in his cap haha 😆

This is the worst rehab ever.. staff ignores every one ..food is always cold ..would not feed it to my chickens

I was unable to walk when I got there and with in 20 days I was up and walking by my own power.Thanks so much for all the help that you gave me.

I was a Patient there from Mid November 2024 to early January 2025 Recovering from a Major Heart Attack and Triple Bypass Heart Surgery. Staff was EXCELLENT, Physical Therapists were EXCELLENT. I Have an upcoming Complete Knee Replacement and can Only Hope they have an Open Bed for Me!! I Highly recommend this Facility! Everyone from the Owner to the Cleaning Staff Genuinely showed Care for the Patients!!

Avoid this place at all costs. Oak River Rehab is one of the most poorly run facilities I’ve ever seen. The staff is miserable, constantly snapping at each other, and the hostility is impossible to ignore. It creates a toxic environment that no patient should ever be exposed to. Therapy here is a complete joke. They show up at 6 AM, rush through the morning, and are gone by around noon. If your loved one isn’t ready the moment they open their eyes, they simply get skipped for the day — no follow-up, no second attempt, nothing. They call this “rehab,” but it’s basically doing the bare minimum and pretending it counts. The most disturbing part is watching residents spend entire days lying in bed or sitting around with zero engagement or stimulation. It’s depressing, neglectful, and absolutely not what a rehab facility should be. If you care at all about your family member, send them anywhere else but here. Oak River Rehab is a disaster. There are places in Redding to seek first. No wonder they are 4 star. Should be 1.

Staff is amazing..they truly care about patients.I was there 2 times in a short period… On top of caring staff..the place is kept so clean.

I was sent to this facility after a bad car accident for physical therapy. The staff are wonderful to me and everyone else even when patients are being awful to them. I appreciate all the support I’ve received here. Highest praise to the facility and staff! Pet friendly as well.

I can’t say enough good things about Oak River. My grandmother broke a hip/leg recently and was here for a few months while recovering and she was treated with care and attention that made the stay as comfortable as possible all things considered. And now my Grandfather’s dementia has progressed to a point that we needed long term care. Terry went above and beyond to make our experience as smooth as possible during a troubling time for our family. Terry helped us in ways we didn’t even know were possible. Thank you Oak River; and Terry you’re the man 🤘. Our family appreciates everything you have done.

If you are looking at the other reviews I will say they have had an absolute turned in their work everything was great here I recommend it. Eveyone was lovely Oak River is 100 percent the place!

Oak River is a phenomenal skilled nursing facility. It is very clean and the staff are friendly and knowledgeable. I highly recommend this facility if you or someone you know is in need of skilled nursing and rehab.

Had the pleasure of speaking with Terry about the facility. The staff is compassionate and caring and that made my heart happy to see that. It’s not an easy decision to place a loved one, but this place would be a great choice should you need it!

Alisha, April,and Brandi are excellent CNA’s.and Amber, Kristi and Hannah night nurses are excellent ..if Qak river was to lose them it would be bad for patients..and Bruce, Jill ,along with the physical therapy team go above and beyond to strengthen and heal. Best place to go in Shasta county to build strength to heal
