Our Facility

Oak River Rehabilitation, located in Anderson, California, provides long- and short-term care, 24/7 skilled nursing, rehabilitation, and respite care in a safe and welcoming space. We offer private rooms, a talented staff, and state-of-the-art equipment and practices. Leadership Team

Our Licensed Nursing Home Administrator (LNHA) directs all facility operations, staffing, services, and programs. The leadership team also includes our Medical Director, who coordinates medical services according to policies and procedures, as well as our Director of Nursing, Rehabilitation Director, Social Services Director, and Environmental Services Director. 

Nursing/Clinical Staff

Led by the Director of Nursing, our nursing staff delivers care and clinical services according to quality standards. Charge Nurses (CN) oversee clinical operations on facility floors and Registered Nurses (RNs) assess, plan, provide, and document nursing care. RNs oversee Certified Nursing Assistants (CNAs), who assist patients with activities of daily living including administering medications, eating, bathing, dressing, and transferring. And Infection Control Nurses (ICNs) help prevent the spread of bacteria, viruses and associated illness in the healthcare environment. 

Therapy/Rehabilitation

Our licensed Physical Therapists (PTs), Speech Therapists (STs), and Occupational Therapists (OTs) create and implement therapy plans according to individual needs. Under the guidance of therapists, Restorative Nurse Assistants (RNAs) assist with regular activities to improve mobility and support recovery. This team is led by our Rehabilitation Director, who guides therapy and rehabilitation strategies, operations, and staff. 

Admissions and Social Services

The Admissions Team guides patients and families in learning about the facility, overseeing insurance information and payments, and navigating agreements and move-in processes, while the Medical Records Team oversees medical files and information. The Social Services Team, led by a Director and supported by Social Workers, leads programs that address mental, social, and emotional needs for patients and families to maximize independence, dignity, and quality of life. 

Dietary Services

Registered Dietitians (RDs) oversee meal planning and presentation to ensure nutritious, tasty meals that fit each patient’s needs.

Environmental Services

The Environmental Services team, led by the Director, oversees facility maintenance and cleanliness according to health and safety standards.
